Principle and conditions
Authorisation to leave the country is obligatory for any minor travelling abroad who is not accompanied by a holder of parental authority. This authorisation must be obtained by means of a form providing the following details:
- The surname (and commonly used surname, if applicable), first and middle names and date and place of birth of the minor authorised to leave the country
- The surname (and commonly used surname, if applicable), first and middle names and date and place of birth of the holder of parental authority who is a signatory to the authorisation and the capacity in which this authority is authorised, domicile, and, if applicable, telephone contact details
- The duration of the authorisation, which must not exceed one year
- The signature of the holder of parental authority
Obtaining and completing the form
To get this form, you can either:

- Or collect it from the Police Department
This form, duly completed and signed, must be accompanied by a legible copy of an official ID document of the signatory.
