How to declare the loss of a document and request a duplicate

Principle and conditions
You must declare the loss of your driving licence, registration plates or annual car sticker in order to request a duplicate.
This procedure will enable you to drive without the lost licence document, plates or annual car tax sticker for eight days, counted from the date on which you declared the loss.

Obtaining and completing the form
To get this form, you can either:
- Download it here
Declaration of loss and request for duplicate (STC040) (385-0023_01-2018)
- Or collect it, on request, from the Driver and Vehicle Licensing Office

Submitting the application
You should take the completed and signed form, with an ID photo (for the driving licence), and one means of payment with you (cash, cheques payable in Monaco or France and Visa bank cards only), to the Driver and Vehicle Licensing Office.
