Principle and conditions

Those who are insured through the State Medical Benefits Office, or those eligible, must inform the State Medical Benefits Office that they are pregnant in order to obtain a maternity record book (carnet de maternité), which will be issued subject to the entitlement to maternity benefits being established.

Obtaining and completing the form

To get this form, you can either:

Undergo the required medical examinations

You should get the following to complete the form, before the end of the third month of pregnancy:

  • The doctor who performs the medical and obstetric examination
  • The doctor who performs the ultrasound examination
  • The medical test laboratory

Sending off the form

You should send the form to the State Medical Benefits Office as soon as possible after you have undergone the required medical examinations.
